Preferred Provider Organization (PPO)
The Preferred Provider Organization (PPO) plans offer the flexibility to choose an in-network or out-of-network provider each time you need care. Keep in mind, you will save money when you visit in-network providers. The PPO plans offer two deductible options. Generally, the higher the deductible, the lower your premiums will be.
High Deductible Health Plan (HDHP)
The High Deductible Health Plan (HDHP) features a higher deductible with the option of participating in a Health Savings Account (HSA). After the annual deductible is met, you pay coinsurance for most health care services, up to the annual out-of-pocket maximum. You have the option to save pre-tax funds in an HSA up to annual limits to pay for you and your family’s health care needs.

Zero Card – NEW Benefit Perk Effective July 1st, 2024
Only available for Plan A and Plan B members. Plan C members are ineligible.
Effective July 1st, 2024 employees and dependents have access to Zero Card, a new benefits program designed to help alleviate medical costs available under the program. Members will have to sign up on the Zero Card website and connect with a Zero Personal Health Assistant to schedule an appointment. The Zero Personal Health Assistant will help locate eligible providers to provide the care that you need at no cost to you. Visit the below link for a list of available services and information about the program.
To sign up, visit their website https://my.zero.health/ and register to start gaining access to the new program. Please note, dependents age 18 and older will have to register their own Zero Card account. Members can also call the below customer service number to register. Members will receive physical Zero Cards 2-3 weeks after the July 1st effective date. Once registered, members have access to a virtual Zero Card to begin utilizing services.
